This department can provide birth and death
certificates.
The State of Georgia's birth and death records
started in 1919. Only Hall County births and deaths are at this
location.
To inquire about births and deaths in other counties
and states, please call (404) 679-4701, 4702, or 4703 for their phone numbers.
Birth Certificates are confidential and are kept in
the county of the birth.
For a child's birth certificate: Only natural
parents, grandparents or legal guardian can purchase the record.
For an adult: Only natural parents, grandparents,
legal guardian, spouse, sibling, or grown child can purchase the
record.
To purchase a birth or death certificate:
1. Rush orders can be federal expressed using
customer's bank card by calling 1(800) 255-2414.
2. Request through mail: The search
fee of $15 (includes first paper copy, if found) is not
refundable. You must send a Money Order for
$15 and $5 for each additional copy to:

For birth certificate requests, include your relationship to the
person you are requesting the record for, their name, date of
birth, mother's maiden name, and father's name.
A legal, legible
picture of the person requesting the birth certificate must be
included (copy, do not send original).
Your phone number and a self addresses stamped
envelope must also be included.

For
death certificates, include your relationship to the person
you are requesting the record for, their name, place of death,
date of death, funeral home (if known), and any other
information that would be helpful in locating the record.
A legal, legible
picture of the person requesting the birth certificate must be
included (copy, do not send original).
Your phone number and a self addresses stamped
envelope must also be included.
3. Come in person to the Hall County Health
Department with your picture I.D. You must have a
legal picture I.D. the search fee of $15 (includes first paper
copy, if found) is not refundable and $5 for each additional copy.
We accept cash, Discover, Master Card or
Visa. No personal checks accepted.
Name changes, Paternity Acknowledgment (adding the
father's name when the parents are not married), and Legitimations
(adding the father's name when the parents were married after the
birth of the child), can be done at this location by appointment
only. Please call (770) 531-5674 for an appointment.
The above fees
have been established in accordance with Chapter 31-10 of the
Official Code of Georgia. Pursuant to O.C.G.A Chapter
31-10-27.

We provide Birth Certificates for persons born
in Dawson County.
We provide Death Certificates for Dawson County residents.

mailing address and telephone number.

Birth Certificate. A legal, legible copy of the person requesting
the records must be included. Also please send money order made
payable to Dawson County Health Department.

Only natural parents, grandparents, legal guardian, spouse, sibling

Death Certificates
Mail to above address. State your relationship to the person
you are requesting the record for, their full name, date of death.
Please include your mailing address and telephone number.
At Home Births:
Please have at
home birth package completed. There will be less wait time, if you
call for appointment and if you want a certified copy, please bring
a money order made out to State Vital Records.
